چکیده
BP: bullous pemphigoid DIF: direct immunofluorescence TNF-a: tumor necrosis factorealpha INTRODUCTION Up to 20% of cases of bullous pemphigoid (BP) show an atypical presentation, exhibiting localized distribution or non-bullous lesions, such as eczematous, urticarial, prurigolike, and erythema multiformelike papules and plaques. These atypical presentations can be associated with a delay in diagnosis and thus require a high degree of suspicion. We present a case of recalcitrant BP localized to the anogenital region with subsequent generalization after administration of etanercept, a tumor necrosis factorealpha (TNF-a) inhibitor. Our review of bibliographic databases did not find any cases with similar distribution and course. TNF-a inhibitors are used in the treatment of pemphigus and pemphigoid. With expanding use of TNF-a inhibitors and longer follow-up periods, there are increasing reports of autoimmune diseases associated with these therapies, including autoimmune bullous disease and systemic lupus erythematosus. This case highlights that caution should be exercised when considering antieTNF-a therapy for autoimmune bullous diseases.

Localized genital bullous pemphigoid; A case report
Bullous pemphigoid (BP) is an autoimmune bullous disorder with urticarial pruritic papules and plaques and tense bullae in flexural surfaces of body. The localized form of the disease is a rare variant which can be triggered by different stimuli. Hereunder, we report a patient with the local type involving genitalia without any triggering factors.

BP: bullous pemphigoid TNF: tumor necrosis factor INTRODUCTION Bullous pemphigoid (BP) is an uncommon skin condition in which erythematous, urticarial papules and plaques progress to form tense bullae, typically in elderly men and women. In this disease, IgG autoantibodies are directed against hemidesmosome proteins, resulting in the formation of subepidermal vesicles. At least 50 drugs have be...

Bullous pemphigoid (BP) is a common pemphigoid disorder, which is localized in approximately 16-29% of cases. A small subset of localized BP cases is associated with prior radiation therapy, most commonly for breast carcinoma. We present a patient with an unusual presentation of localized BP after receiving partial accelerated breast irradiation (a type of brachytherapy that has a decreased amo...

The association between psoriasis vulgaris and bullous pemphigoid is due to the still unclear autoimmune process. The common disease site is the dermo-epidermal junction or basal membrane zone (BMZ), with specific alterations for both diseases. Photochemotherapy (PUVA) is one of the therapeutic modalities for psoriasis and can trigger production of autoantibodies against antigens in the BMZ in ...

Background: Tumor necrosis factor-alpha (TNF-α) is a proinflammatory cytokine that plays an immunomodulatory role in a variety of systemic and dermatologic disease. Its blockade can be achieved by using specific inhibitors like etanercept. At present, etanercept is approved for rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, juvenile idiopathic arthritis and psoriasis treatme...
